Assigns ICD-9-CM and CPT-4 codes to patient records based upon physician documentation.
Responsibilities: Abstracts medical records according to Department procedures while maintaining 95-100% accuracy on a daily basis. Assigns ICD-9-CM diagnosis and procedure codes. Accurately sequences codes according to UHDDS guidelines, basic coding principles and hospital specific coding guidelines. Updates coding books to reflect changes in coding guidelines. Demonstrates a thorough working knowledge of medical terminology, the process of disease, computer applications, and the ability to research the reference library. Performs diagnosis and procedure coding. Assigns CPT-4 procedure codes. Validates the accuracy and sequencing of assigned ICD-9-CM codes to assure the correct DRG is assigned to each case on a daily basis. Assigns the APC to applicable Medicare outpatient service records.
Qualifications: High school diploma or equivalent required. Coding certificate course is required. Strong background in medical terminology, disease process, ICD-9-CM, CPT-4 coding required. CCS, CPC or RHIT eligible required.
