Description: Neshaminy Manor Nursing Home, (The Bucks County Nursing Home) located in Warrington, PA is looking for an energetic Caseworker. The Caseworker will provide Social Work services addressing the psychological, emotional, and behavioral needs of an assigned caseload within our long-term care (360) resident population.



The ideal candidate will have excellent written and verbal communication skills and will be responsible for timely documentation and completion of all resident assessments.



The Caseworker will also be involved in the admissions process, care planning, Behavior Management, and discharge planning. When applicable, the Case Manager will provide on-going educational and supportive programs our residents, staff, families, and community as needed.





Applicants must meet the following requirements:



1 year experience in Long Term Care.

A Bachelors Degree in Social Work.

Behavior Management exprierence preferred.

Computer skills are needed for this position.

Applicants must be able to work every fourth weekend.

Applicants will be required to take a skills test.
